You may apply for SNAP by mail, in person, or online at mybenefits.ny.gov.

• Completed applications can be faxed to (716) 858-6834.
• Completed applications can be mailed to:

- Erie County Department of Social Services

- Buffalo, NY 14201

- Attention: SNAP Division

Once we receive your signed application, we will schedule an appointment for a phone of in-office interview. You will be notified of your appointment date through the mail or phone.

Submit your completed and signed application (LDSS-4826) with copies of the following documents listed below. Please do not send original documents, only copies are required.

• Verification of Identity of the person submitting application (Birth Certificate, etc.) and any authorized representative who may be applying on your behalf.
• Verification of current resident and heating expenses.
• Verification of Earned and Unearned Income; including Wage stubs for the past four (4) weeks or tax forms and/or ledgers to verify self-employment; or Monthly VA Benefits, Pension, Alimony and Child Support, Compensation or Disability payments, etc.

If you have difficulty obtaining needed documents for verification, our Agency will assist you.
